If you have a prescription that needs to be refilled --- and is not a narcotic or a sedative -- you may request the refill through our website by completing and submitting the form below.

When we receive your request, we will check your patient chart and ask the doctor or nurse practitioner to approve your request. We will e-mail or call you and let you know either that your prescription has been called in to the pharmacy or if it will be necessary for the doctor to speak with you prior to approving your request
